Due to COVID-19 all patients over the age of 10 must wear a mask upon entering the building. If you are not able to wear a mask, have symptoms of fever, cough, or shortness of breath, or have had potential exposure to COVID-19, please remain in your vehicle and call our office at (501) 843-4555. Our staff will instruct you on what to do prior to entering our building. We are doing COVID-19 testing in our facility, but with varying turn-around times. 



  • MASK MANDATE: Effective 04/19/2021, with guidance from the Arkansas Department of Health, we will continue to require everyone entering our facility to wear a mask. However, we will no longer be screening everyone with a temperature check or questions upon entering the building. If you are concerned that you may have COVID symptoms, please call our office prior to entering at (501) 843-4555 to see how we can best serve you.
  • PORTAL: Please contact our office to get set up with your patient portal today. Click here for patient instructions. 
  • PHREESIA: For a more hands-free check-in process, make sure to complete your Phreesia registration before you arrive for your appointment. You will receive your notification via text or email. If you do not prefer this method, please arrive to your appointment 20-30 minutes early to do your paperwork in the office. Please ask any of our receptionists for more information.
  • PFAC: Cabot Medical Care now has a Patient and Family Advisory Council (PFAC) that we would love for you to be a part of. This is a group of individuals (CMC patients and/or family of CMC patients) that want to partner with CMC in positively shaping our patient experiences. Please email nford@cabotmed.com for more info. 
  • WELLNESS: Most insurances cover wellness' for most ages. This is recommended for all patients to have done yearly. Please call our office to see if you are due for yours.
